The Middlelayers, Inc. website is a C3S-Ai-driven Case Management, Care Coordination System Platform for Population Health. SMS Communication & Consent

As part of our care coordination services through the C3S.ai platform, we may send you SMS (text) related to your services, including appointment reminders, referral updates, and case-related notifications. These messages are sent only to individuals who have provided prior consent, either verbally or in writing, during the intake process through a participating agency.

Message frequency may vary depending on your level of engagement, typically ranging from 1 to 4 messages per month. Standard message and data rates may apply. You may opt out of receiving these messages by replying “STOP.” For additional assistance, you may reply “HELP.”

We do not send promotional or marketing messages, and your mobile number will never be sold or shared with third parties for advertising purposes.

 

Automatically Collected Information

Information about your computer hardware and software may be automatically collected by Middlelayers, Inc.. This information can include: your IP address, browser type, domain names, access times and referring website addresses. This information is used for the operation of the service, to maintain quality of the service, and to provide general statistics regarding use of the Middlelayers, Inc. website.
